Below is the initial application form for the VTS Ophthalmology Process. This exam will be held in the fall of 2024. The testing date will occur remotely within a few weeks prior to the AVOT/ACVO meeting in Savannah, GA. Please fill this form out and submit it between January 1st, 2024 and February 28th, 2024.  Please fill out this form AFTER you have read the requirements listed in the bylaws and feel you meet those requirements.  This initial application is to ensure that you meet all of the initial requirements. Listed below is what you will be required to provide during this initial application.

-Two Letters Of Reference—Either 2 letters from DACVO, one letter from a VTS (Ophthalmology) and one letter from a DACVO, or one letter from a DACVO and one letter from a Supporting Veterinarian or a Diplomate of another Veterinary College deemed appropriate by the Board of Regents such as DACVIM, DACVS, or other. This letter can be either provided by the author directly (preferred) or by the applicant to avot_vts@yahoo.com.

  1. Read through the items listed on the credentialing page. Be sure that you understand all of the requirements for application before applying.

  2. Obtain your two letters of recommendation. These will need to be uploaded to your initial application.

  3. Prepare all other required documents, including your proof of license, proof of NAVTA membership, and proof of CE. These will all need to be uploaded to your initial application.

  4. Once your information has been received you will be contacted by the AVOT. If you do not provide ALL of the requested information your application will be rejected. *
